What Is a PCOS Assessment Scan?

Polycystic Ovary Syndrome (PCOS) is an endocrine (hormonal) disorder affecting women of reproductive age. It is one of the most common hormonal conditions in women and a leading cause of fertility issues.

PCOS occurs when the body produces too many androgens (male hormones), which can interfere with ovulation and cause a range of symptoms. Many women with PCOS develop small fluid-filled follicles on their ovaries — giving them a "polycystic" appearance on ultrasound.

Important: Not all women with polycystic-appearing ovaries have PCOS, and some women with PCOS have normal-appearing ovaries on ultrasound. PCOS is diagnosed based on a combination of symptoms, blood tests, and ultrasound findings.

What We Assess With This Scan

  • Both ovaries (size, volume, follicle count and distribution)
  • Uterus
  • Endometrium (lining of the womb)
  • Surrounding pelvic structures

This scan cannot assess:

  • Hormone levels (blood tests are required for this)
  • Definitively diagnose PCOS (requires clinical assessment, blood tests, and ultrasound together)

  • Irregular periods or no periods
  • Heavy periods when they occur
  • Difficulty getting pregnant
  • Excess facial or body hair (hirsutism)
  • Thinning hair or hair loss on the scalp
  • Acne, especially along the jawline
  • Weight gain, particularly around the abdomen

  • Endometrial thickening (in women with irregular periods)

What to Expect

Before Your Scan

Full bladder required for the first part of the scan: Drink 500ml-1 litre of water starting about 1 hour before your appointment. Best timing for the scan: If you have periods, the scan is ideally performed early in your cycle (days 3-5). If your periods are irregular or absent, the scan can be performed at any time.

Preparation checklist:

  • Drink 500ml-1 litre of water 1 hour before appointment
  • Wear comfortable, easily removable clothing
  • Arrive 10 minutes before your appointment time

During the Scan

The scan begins with a transabdominal ultrasound with a full bladder to provide an overview of the pelvic structures. For the most accurate assessment of the ovaries and follicle count, a transvaginal ultrasound is recommended. You'll be asked to empty your bladder. A thin probe, covered with a sterile sheath, is gently inserted. This provides detailed views of the ovaries and allows accurate follicle counting. The scan typically takes 20-30 minutes.

After Your Scan

You can return to normal activities immediately. There are no restrictions.

Results & Next Steps

Your scan is reviewed by a consultant radiologist, who prepares a formal report. This report is sent to your referring doctor within 2-3 working days. Remember: The ultrasound findings are just one part of the PCOS diagnosis. Your doctor will interpret the results alongside your symptoms and blood tests.
