What Is a Ovarian Ultrasound Screening?

An ovarian ultrasound is a detailed gynaecological examination of the ovaries and surrounding pelvic structures using advanced 2D and 3D imaging. This scan is used to screen for and assess ovarian cysts, tumours, and other abnormalities.

Some ovarian conditions—including cancer—can be hard to detect early because symptoms may be subtle. An ultrasound scan provides a detailed look at the ovaries and can help guide next steps if anything unusual is seen.

What We Assess With This Scan

  • Both ovaries (size, shape, internal structure)
  • Any cysts or masses (simple, complex, solid)
  • Free fluid in the pelvis
  • Uterus
  • Surrounding pelvic structures

This scan cannot assess:

  • Definitively diagnose cancer (blood tests and specialist referral may be needed)
  • Assess genetic mutations (genetic testing is separate)

  • Ovarian tumours (benign or concerning)

What to Expect

Before Your Scan

Full bladder required for the first part of the scan: Drink 500ml-1 litre of water starting about 1 hour before your appointment. Finish drinking at least 20 minutes before your scan time.

Preparation checklist:

  • Drink 500ml-1 litre of water 1 hour before appointment
  • Wear comfortable, easily removable clothing
  • Arrive 10 minutes before your appointment time

During the Scan

The scan begins with a transabdominal ultrasound (probe on your lower abdomen) with a full bladder. For more detailed views of the ovaries, a transvaginal ultrasound is usually recommended. You'll be asked to empty your bladder. A thin probe, covered with a sterile sheath, is gently inserted to provide much clearer, closer images of the ovaries. The scan typically takes 20-30 minutes.

After Your Scan

You can return to normal activities immediately. There are no restrictions.

Results & Next Steps

Your scan is reviewed by a consultant radiologist, who prepares a formal report. This report is sent to your referring doctor within 2-3 working days. If an abnormality is found, your GP may recommend blood tests (such as CA125), referral to a gynaecological oncologist, or additional imaging.

The transabdominal scan is completely painless. The transvaginal scan may cause mild discomfort but should not be painful. Let your sonographer know if you feel uncomfortable at any point.
The complete examination takes approximately 20-30 minutes.
A GP referral is recommended. If you don't have one, please contact us to discuss your situation.
Ultrasound can identify suspicious features in the ovaries, but it cannot definitively diagnose cancer. If concerning features are found, your doctor will recommend further tests.
CA125 is a blood test that measures a protein that can be elevated in ovarian cancer. However, it can also be elevated by other conditions, so it's used in combination with ultrasound and clinical assessment.
Your images are reviewed by a consultant radiologist and a formal report is sent within 2-3 working days.
Many ovarian cysts are functional (related to the menstrual cycle) and resolve on their own. Your doctor will assess the characteristics of the cyst and advise whether monitoring or further investigation is needed.
