What Is a Male Pelvic Ultrasound Scan?

A male pelvic ultrasound is a non-invasive examination of the male pelvis, focusing on the urinary bladder and prostate gland. This scan uses sound waves to create detailed images, helping to assess bladder health, prostate size, and how well the bladder empties.

The scan is performed transabdominally (through the lower abdomen) and includes a post-void measurement — where you empty your bladder and return for additional imaging to check how much urine remains. This is an important indicator of bladder function.

What We Assess With This Scan

  • Urinary bladder (size, wall thickness, any abnormalities)
  • Prostate gland (size and structure)
  • Post-void residual volume (urine remaining after emptying bladder)
  • Lower ureters (when indicated)

This scan cannot assess:

  • Definitively diagnose or rule out prostate cancer (MRI or biopsy may be needed)
  • Assess PSA levels (blood test required)

What to Expect

Before Your Scan

Full bladder required: Drink approximately 1 litre of water starting about 1 hour before your appointment. Finish drinking at least 20 minutes before your scan time to allow your bladder to fill. Try not to urinate before the scan.

Preparation checklist:

  • Drink 1 litre of water 1 hour before appointment
  • Avoid urinating before the scan if possible
  • Wear comfortable, easily accessible clothing
  • Arrive 10 minutes before your appointment time

During the Scan

You'll lie on an examination table. The sonographer applies warm gel to your lower abdomen and gently moves the ultrasound probe across your skin to capture images of your bladder and prostate. You'll then be asked to go to the bathroom and empty your bladder as completely as possible. When you return, the sonographer will take additional images to measure your post-void residual. The scan typically takes 20-30 minutes.

After Your Scan

You can return to normal activities immediately. There are no restrictions or downtime.

Results & Next Steps

Your scan is reviewed by a consultant radiologist, who prepares a formal report. This report is sent to your referring doctor within 2-3 working days. A normal post-void residual is typically less than 50ml. Elevated residuals may indicate incomplete bladder emptying.

Prostate enlargement (benign prostatic hyperplasia) is very common in men over 50 and is usually not serious. Your GP will discuss the findings and any treatment options if needed.
